Add Hospice Physician metrics to Mosaic

What: Add Physician hospice metrics to Explore and Analyze pages. And add the Hospice timing table to the physician analyze page (Includes: Hospice LOS & Mortalities and Hospice Timing - Last 30 Days of Life)

These metrics are already in Marketscape-Hospice but we don't have them in Mosaic.

(FYI - We have all the metrics below for physicians for SNF and HHA but Hospice was missing.) This will help our ACO clients understand areas of improvement around hospice which is becoming a bigger issue/goal.
