ACO Patient Path days to next site of care

Within Patient Path, if a pt goes home from the hospital and home health does not start in two days, home health does not look like the next site of care. Realistically, it is and there are many reasons why home health may not start within 2 days on the claims. The doctor can approve a later start; the patient has dialysis or other appointments; etc. This also affects the 3rd tier of site of care. If a pt goes home from the hospital without home health, readmits to the hospital and goes back home, we loose sight of home health being ordered if they don't start care in 2 days because the Path only has 3 tiers.
