Changes to the Network Add/Edit tool

1) Change the top circles to navigation tabs.

2) Change the bottom navigation to Back, Next Done

3) Remove Summary - it only shows what is on the first page

4) Add provider type icon to help differentiate provider type on Add/Remove Providers tab

5) Change name of Add/Remove tab from Add Providers to Add/remove Providers since both tasks can be done there.

The purpose and priority for this change is tied to ease of use.
